4. Services and Subscriptions

We offer monthly and quarterly subscriptions for Continuous Glucose Monitors and related products. Subscriptions auto-renew unless cancelled before the renewal date. No refunds for unused subscription portions upon cancellation.

5. Payment and Refund Policy

Accepted payments: credit/debit cards, EFT, and insurance billing. We offer a 14-day return policy for all shipped Medical Equipment. Contact us within 14 days of delivery to initiate a return per Medicare guidelines.

6. Dispute Resolution

Governed by the laws of the State of Texas. All disputes resolved via binding arbitration in the Northern District of Texas. Class action waivers apply. Claims must be filed within one (1) year of arising.

7. Intellectual Property

All content is owned by EquipMed Global, LLC or its licensors. No reproduction or distribution without prior written permission. User-generated content grants us a royalty-free worldwide license.

8. Limitation of Liability

Services provided "as is." Maximum liability capped at amounts paid in the last 6 months or $100, whichever is greater. No liability for indirect, incidental, or consequential damages.

9. Data Protection

Your data is governed by our Privacy Policy. We implement reasonable security measures. We are not responsible for third-party service privacy practices.

10. Termination

We may suspend or terminate your account at any time for breaches of these Terms. Upon termination, all licenses cease and stored credits are forfeited. Fees are non-refundable.

12. Force Majeure

Neither party is liable for failure to perform due to events beyond reasonable control. Affected party must notify in writing within 14 days of onset.

14. Opt-In & SNS Authorization

By submitting any form, you grant permission to be contacted via phone, email, or text. We may perform a Same or Similar (SNS) check. If a similar HCPCS code was billed in the past 5 years, Medicare may deny the claim. Products will not be shipped in case of SNS failure unless you agree to self-pay.
